We look forward to working with you over the next several months. We hope this information will be helpful in working with us.

When scheduling a monitoring visit, the more notice we have, the easier it is to accommodate your schedule. Available space and time fills quickly and it can be difficult to guarantee availability with one week's advance notice. Please try to schedule visits as far in advance as possible.
To facilitate preparation for your visit and enhance the time you are here, please let your study coordinator know the following prior to your visit:
| |

What information will you be reviewing at this visit (i.e. specific charts, specific visits, regulatory documents)? This will allow us to have everything ready for your visit. |
| |

If there will be someone accompanying the primary monitor, your study coordinator will need to know in order to provide adequate space. |
| |

Do you need a data line available? |
| |

Will you need the Principal Investigator at this visit? |
| |

How much dedicated time will you need with the study coordinator? This will allow us to schedule patients accordingly. Coordinators will be providing patient care throughout the day, but with advance scheduling, we make every effort to provide dedicated time for corrections. |
